Incident[edit] Atchisonhandledaroutinetake-offat08:20localtime(07:20UTC)thenhandedcontroltoLancasterastheplanecontinuedtoclimb.BothpilotsreleasedtheirshoulderharnessesandLancasterloosenedhislapbelt.At08:33(07:33UTC)theplanehadclimbedthroughabout17,300feet(5,300 m)[4]: 3 overDidcot,Oxfordshire,andthecabincrewwerepreparingformealservice. FlightattendantNigelOgdenwasenteringthecockpitwhentherewasaloudbang[5]andthecabinquicklyfilledwithcondensation.Theleftwindscreenpanel,onLancaster'ssideoftheflightdeck,hadseparatedfromtheforwardfuselage;Lancasterwaspropelledoutofhisseatbytherushingairfromthedecompressionandforcedheadfirstoutoftheflightdeck.Hiskneeswerecaughtontheflightcontrolsandhisuppertorsoremainedoutsidetheaircraft,exposedtoextremewindandcold.Theautopilothaddisengaged,causingtheplanetodescendrapidly.[5]Theflightdeckdoorwasblowninwardontothecontrolconsole,blockingthethrottlecontrol(causingtheaircrafttogainspeedasitdescended),flightdocumentsandchecklistsweresuckedoutofthecockpitanddebrisblewinfromthepassengercabin.OgdenrushedtograbLancaster'sbelt,whiletheothertwoflightattendantssecuredlooseobjects,reassuredpassengers,andinstructedthemtoadoptbracepositionsinanticipationofanemergencylanding. Theplanewasnotequippedwithoxygenforeveryoneonboard,soAtchisonbeganarapidemergencydescenttoreachanaltitudewithsufficientairpressure.Hethenre-engagedtheautopilotandbroadcastadistresscall,buthewasunabletoheartheresponsefromairtrafficcontrolbecauseofwindnoise;thedifficultyinestablishingtwo-waycommunicationledtoadelayininitiationofemergencyprocedures. Ogden,stillholdingontoLancaster,wasbynowbecomingexhausted,sopurserJohnHewardandflightattendantSimonRogerstookoverthetaskofholdingontothecaptain.[6]BythistimeLancasterhadshiftedseveralcentimetresfartheroutsideandhisheadwasrepeatedlystrikingthesideofthefuselage.Thecrewbelievedhimtobedead,butAtchisontoldtheotherstocontinueholdingontohim,outoffearthatlettinggoofhimmightcausehimtostriketheleftwing,engine,orhorizontalstabiliser,potentiallydamagingit. Eventually,AtchisonwasabletoheartheclearancefromairtrafficcontroltomakeanemergencylandingatSouthamptonAirport.TheflightattendantsmanagedtofreeLancaster'sanklesfromtheflightcontrolswhilestillkeepingholdofhim.At08:55localtime(07:55UTC),theaircraftlandedatSouthamptonandthepassengersdisembarkedusingboardingsteps.[7] Lancastersurvivedwithfrostbite,bruising,shock,andfracturestohisrightarm,leftthumb,andrightwrist.[5][8]Ogdenhadcutsandbruisestohisarm,andlaterPTSD.Therewerenootherinjuries.[8] Investigation[edit] Policefoundthewindscreenpanelandmanyofthe90boltssecuringitnearCholsey,Oxfordshire.[4]: 12 Investigatorsfoundthatwhenthewindscreenwasinstalled27hoursbeforetheflight,84oftheboltsusedwere0.026inches(0.66 mm)toosmallindiameter(BritishStandardsA211-8CvsA211-8D,whichare#8–32vs#10–32bytheUnifiedThreadStandard)andtheremainingsixwereA211-7D,whichisthecorrectdiameterbut0.1inches(2.5 mm)tooshort(0.7inchvs.0.8inch).[4]: 52 Thepreviouswindscreenhadalsobeenfittedusingincorrectbolts,whichwerereplacedbytheshiftmaintenancemanageronalike-for-likebasiswithoutreferencetomaintenancedocumentation,astheplanewasduetodepartshortly.[4]: 38 Theundersizedboltswereunabletowithstandtheairpressuredifferencebetweenthecabinandtheoutsideatmosphereduringflight.
